Current enforcement of 6-month passport expiration and 90-day visa?

Our travel plans have been fluid since arriving in Ecuador. We’ve done our share of research on Google, but would appreciate anyone’s recent experience or knowledge of the following:
• How strict is Ecuador about entering with a passport that expires in less than 6 months (it is a USA passport)? Is there a way to get a letter of approval to re-enter? One of us has a passport that expires in May and didn’t think enough in advance to realize we were taking a trip outside the country in January and would need to re-enter. We’d rather not go through the hassle of renewing it while here if we don’t have to.
• What is the current consequence of staying beyond 90 days without a visa? We’ve read reports that they no longer enforce a fine and just require getting a visa if returning within a year. We don’t intentionally plan to do this, but want to know the consequence if we would end up stays past the magic 90-day date.
